How does it work in the body?

Mesalazine is the active part of sulfasalazine in the treatment of ulcerative colitis. It acts topically on the intestinal wall in inflammatory bowel disease, although its exact mechanism of action is not clear.

Increased leukocyte migration, abnormal cytokine production, increased production of arachidonic acid metabolites, particularly leukotriene B4 and increased formation of free radicals in inflamed intestinal tissues are all present in patients with IBD. Mesalazine has in vitro and in vivo pharmacological effects that inhibit white cell chemotaxis, reduce cytokine and leukotriene production and scavenge free radicals. It is currently unknown if any of these mechanisms play a major role in the clinical efficacy of mesalazine.

Dosage and method of use

Adults: For an enema, the recommended dose is one enema at bedtime.

As for the tablets, one tablet is taken at a concentration of 500 mg 3 times during the day.

Children: Not recommended.


Patients with known sensitivity to salicylates or any of the excipients.
Patients with severe hepatic and/or renal insufficiency.


What are the reasons for using the special categories?

Pregnancy and lactation
should be used with caution during pregnancy and lactation and only if, in the opinion of your physician, the potential benefits outweigh the potential risks.

Mesalazine crosses the placental barrier. There are no adequate data on the use of PENTASA in pregnant women. However, data on a limited number of exposed pregnancies indicate that there is no adverse effect of mesalazine on pregnancy or on the health of the fetus/newborn. To date no other relevant epidemiological data are available.

In one case after long-term use of a high dose of mesalazine (2-4 g, by mouth) during pregnancy, renal failure was one of the outcomes.

Animal studies of oral mesalazine do not indicate direct or indirect adverse effects related to pregnancy, fetal/fetal development, parturition, or postnatal development.

In cases of breastfeeding

N-acetyl-5-aminosalicylic acid and to a lesser extent mesalazine are excreted in breast milk. The concentration of mesalazine in breast milk is lower than that in maternal blood, while the metabolite acetylmesalazine appears in similar or increased concentrations. There is limited experience of using oral mesalazine in nursing mothers available to date. There are no controlled studies with PENTASA during breastfeeding. Hypersensitivity reactions such as diarrhea in an infant are very dangerous. Therefore, use PENTASA only while breastfeeding, if the potential benefits outweigh the potential risks. If the infant develops diarrhea, breastfeeding should be discontinued.
